Choosing a new provider is an important decision. At EverWell Health, you can expect compassionate, personalized care in a comfortable setting where your questions are heard and your health goals matter. Before your visit, bring a valid photo ID, your insurance card if applicable, and a complete list of medications, vitamins, and supplements. Bring relevant medical records if available, a list of your questions and concerns. Please arrive 15 minutes early to complete any necessary paperwork. Insurance participation and coverage may vary. We currently accept Medicaid, Medicare, Anthem, UHC, Tricare, Cigna, Connecticare, and Aetna. Self-pay options are also available. Ask about our EverWell SimpleCare Program: No Insurance No Problem.

Appointment policy: EverWell Health provides care by appointment to allow each patient dedicated, unhurried time. If you need to cancel or reschedule, please contact the office as early as possible. Missed appointment fee $50 for less than 24 hours' notice. EverWell Health does NOT provide emergency services. If you are experiencing chest pain, difficulty breathing, signs of a stroke, uncontrolled bleeding, or another medical emergency, CALL 911 or go to the nearest emergency department.
